- Call Dr. Jayakumar’s office to schedule a consultation.
- At the time of consultation a detailed history and physical examination will be completed.
- Further tests will be ordered if you meet the medical criteria, to ensure that your overall health is good enough to endure the surgery. Dr. J may recommend a psychological evaluation to assess your mental status and competency to comply with the lifelong changes of the procedure.
- Verification of insurance coverage:
- Predetermination: to determine if the procedure covered under your plan. The consultation report will be submitted to the insurance company.
- Preauthorization: after predetermination authorization is requested and surgery is scheduled. Approximately 3-4 weeks for surgery date.
- Preoperative visit: second visit to the surgeon’s office. Preoperative instructions and consent forms are given. You are encouraged to ask any remaining questions prior to scheduled surgery date.
